The 16th Annual Cacowards, also referred to as Cacowards 2009, was the sixth annual Cacowards feature, released by Doomworld on December 11, 2009. It was written by Stephen Browning (Scuba Steve), Patrick Pineda, Ryan Nematollahi (hobbs), and Michael Mancuso (esselfortium), and illustrated by Mike Lightner (Mancubus II), with a foreword by Bill Koch (Bloodshedder).

This edition had two runners-up. It started the "I'm Bored!" sidebar segment, in which readers were asked to create a short Doom map before New Year's Eve, based on the output of a random idea generator.
