The baron of Hell returns as a monster in Doom Eternal, retaining its appearance from Doom (2016), but now equipped with a pair of flaming blades, similar to those previously wielded by the cyberdemon. It is currently presumed that it will have statistics mostly matching those from the previous game, but further information from id Software is needed. In addition to its fireballs, it can now perform a series of sword slashes and a jump attack that releases a fiery shockwave upon impact.

Only a few Barons have been seen in any explicit amount of detail. The ones seen in the E3 teaser seem to have the same pinkish complexion as their predecessors, however the one fought in the QuakeCon gameplay reveal was an ashy grey-brown with fiery eyes and inner flesh that glowed like embers. Whether this is the final design or a potential variant is unknown. The Slayer was shown being able to knock it back with a melee strike, potentially meaning the demon could be slightly weaker, or could entail that the Destructible Demons gore system could potentially reduce the demon's weight, making them more susceptible to stunning.

As far as combat goes, it seemed to fight on par with the 2016 variant, but with the addition of swipes with its new argent blades. Additionally, all of the once green effects associated with the demon have been replaced by fiery orange and red ones.
