The pain elemental returns as a monster in Doom Eternal, its first appearance in a main-series Doom game since Doom 64, and in any Doom game since Doom RPG. It retains the same appearance as its prior incarnation in Doom II: a round, flying, brown enemy with a single large red eye, two black horns, two muscular arms, and a large mouth like that of the cacodemon. It is currently presumed that it will have statistics and behaviors which mostly match those from Doom II, but further information from id Software is needed. It retains its ability to spawn lost souls, and can also use them as a direct attack by throwing them at the Doom Slayer.

The Doom Slayer uses the Doom blade to stab the pain elemental and rip its eye out. After clawing at its empty eye socket for a short time, the monster explodes.

The Doom Slayer punches then stabs the Pain elemental in the eye.

Battlemode

The pain elemental is one of the five playable demons in the Battlemode multiplayer game mode. Its attacks are throwing a lost soul directly at the player, and using idle lost souls as a shield mechanism to protect itself from oncoming attacks from the Slayer.

Pre-release

The pain elemental was first confirmed to exist in the teaser trailer released at E3 2018, and was later shown in full at QuakeCon.
