It DOES modify the engine, and it is technically a commercial game using the Doom Engine. The fact this article is of low quality would make me say "delete," but actually we should have Chex Quest material on the wiki. What we've learned about it since that was written (stuff about the fact it does modify engine behaviors and isn't just a DEH patch, for example) is an important factor, I think.
