Death Tormention II (pe4_dt2.wad) is an episode replacement for The Ultimate Doom designed by Paul Corfiatis, Kristian Aro (Nebula) and Chris Harbin, released on November 21, 1999. It is a sequel to Death Tormention and has its own sequel, Death Tormention 3. Aside from replacing all nine levels of Thy Flesh Consumed, this WAD also changes the secret levels from the other three episodes as a bonus.
In 2019, Death Tormention: The Complete Trilogy was released, which contains the levels from Death Tormention II in E3Mx slots (some were tuned up). However, the bonus secret levels were cut.

- E4M2 is a modified version of MAP24 from The Twilight Zone II.
- E4M5 was previously released standalone as Death Tormantion, Special edition level [sic].
- Paul Corfiatis and Chris Harbin used the following utilities: DoomCAD v6.1, DETH v4.16, BSP v3.0, WinTex v4.1. Kristian Aro used DeeP97.
