E1M3: Toxic Treatment is the third map of the first episode in Return to Hadron. It was designed by Matt Powell (cannonball) and uses the music track "Smoke and mirrors" from Rise of the Triad (original episode) or "Stargaze" by Psyrus (Ultimate edition). In the Ultimate edition this level received significant changes in both geometry and things population.

Secrets[edit]
- From the start, go right and use the wall with the poison symbol on it to reveal a hallway, eventually taking you to a chaingun and a berserk pack. (sector 29 in original episode, sector 110 in Ultimate edition)
- In the red key room, use the computer panel to the left of the switch to lower a lift in the southeast corner of the room. Run to it quickly to retrieve a soul sphere. (sector 109)
- From Secret #4, there is a shootable switch to the east. Shoot it for a block to rise, allowing you to go onto the next platform, which contains the blue key, a radiation suit, and some armor bonuses. (sector 115)
- After getting the red key, open the red door near Secret #2 to find a lift switch, but look towards the computer terminals on the right wall to find a tucked-away switch. Use this one to reveal a switch in Secret #2, then go back there and flip that switch for a lift to the south to lower. Take this lift up for a megaarmor and some health bonuses. (sector 260)
- Original episode: From where the yellow key is located, look towards the fountain to see a tiny step allowing you to get onto the fountain. From there, jump west and outside where you can collect health and armor bonuses as well as a computer area map. (sector 278)
Ultimate edition: At the north end, go to the water at the west and flip the switch, which reveals a computer area map to the south. (sector 456)
