E1M4: Medlabs is the fourth map of Alpha Accident: Terra Nova. It was designed by Eugene Guschin (Wraith777) and uses the music track D_COUNTD by Paul Corfiatis (pcorf), from Pcorf's Music Rejects. The par time defined in ZMAPINFO is 1:30.

Secrets[edit]
- Head northwest from the start, then head behind the wall that lowers and open the left terminal to find ammo clip, armor shard, shotgun shells, health bonuses, and a box of rockets. (sector 569)
- Head northeast at the start and blow up the destructible column to the east. Afterwards, head back to the entrance of this area and go to the box of ammo at the east wall. Doing this lowers the platform where the destructible column was. Run back there and ride it up to get armor bonuses, shotgun shells, and rockets. (sector 567)
- Open the west wall in Secret #2 and head north in the corridor to two rooms to the left and right. Flip the switch to the right, then enter the room east of there and take the teleporter to head to the northwest part of the map. (sector 497)
- In Secret #3, lower the terminal and destroy the destructible column to the west. Head through the corridors west of it, opening the wall at the southernmost end, then heading east. Shoot another indestructible column and head down the lift where it is on, then head south. Another lift later and you will find a passage leading to the outside area at the west side. (sector 565)
- In the outside area, head south on the first step and use it to enter the passage with a stimpack and shotgun shells. (sector 399)
- In the southwest hall, shoot a container at the south wall and open the wall to find a passage leading east and north to a stimpack, shotgun shells, two destructible columns with lifts leading down, and an energy cell pack. (sector 333)
- At the entrance of Secret #6, open the first recessed wall at the south end to find a container. (sector 607)
- In the central hall, enter the large room southwest of it and into the small inner part. Flip the switch inside to lower a lift to the west. Hug the north wall, and as the lift rises, you will enter a small corridor with partial invisibility and shotgun shells. (sector 55)
- Head back to the central hall and head south, then east into another blue hall. Shoot the nearby destructible column and take the lift it is on down to several corridors with shotgun shells, a stimpack, and an energy cell. (sector 207)
- In Secret #9, head near the north side where the stimpack is, then lower the west wall. Ride the lift to get a berserk pack. (sector 181)
