Essentials[edit]

Step forward to collect a shotgun under your feet, kill the shotgun guys (and lost souls on Hurt Me Plenty (HMP) skill level or higher) around you then go to the east end of the room and climb up the steps to a wooden door. Open the door and kill any cacodemons waiting behind it (one will teleport behind you), then drop into the blood pit behind the door and approach the south wall to trigger an ambush by (depending on skill level) two demons, Hell knights or revenants. There are two skull switches on the south wall that require keys to activate, and an eye switch will open between them after a short delay; shooting the switch will lower the column at the north end of the pit to reveal a teleporter, but you will have to fight off imps that teleport in while you wait. Once the teleporter is revealed, enter it to teleport back to a block near the starting area and get a supercharge and a super shotgun.

As you step off the block, part of the south wall will open and reveal a compartment with chaingunners and cacodemons, along with two imps, revenants or arch-viles - one of them will teleport to a block to the east to flank you. Go inside the compartment to get a rocket launcher, which will open a cage at the north end of the room with many chaingunners and imps, then look through the bars of the cage to the north-west corner to see a skull switch that you can shoot to lower the bars in the compartment, letting you reach the blue keycard. Taking the blue key opens two cages to the east and west, releasing two to six pain elementals that will teleport into the starting area. Fight your way through the starting area to the ledge on the east side of the blood pit, and collect the yellow keycard placed between the bars of the cage.

Drop into the blood pit and press both skull switches on the south wall, which will open a compartment to the east with cacodemons and chaingunners (plus revenants on HMP or higher). Pull the skull lever inside the compartment, which will cause shotgun guys to appear overhead, then use the teleporter to go back to the starting point, where the red cage by the starting point has opened - the upper set of bars are crushers, and you must carefully step through them, as well as the lower set of bars which move up and down in a pattern, to get the red keycard hidden inside the cage. Now enter the compartment where you found the blue key and open the wall in the south-west corner, which is indicated by a red strip, then look at the right-hand wall to see a tiny eye switch in a recess that you can shoot to raise stairs leading up to a BFG9000 and a face switch.

Press the face switch then go back to the starting area, where the east wall has lowered to reveal imps, as well as two arch-viles and a cyberdemon on HMP or higher - on Ultra-Violence and Nightmare, a third arch-vile will teleport to the north cage and start resurrecting the monsters there. At the back of the east cage is a skull switch that must be shot several times to detonate some unseen barrels behind it, activating a voodoo conveyor and lowering the blocks in the corners of the room to reveal two more skull switches guarded by caged chaingunners (or arch-viles on HMP or higher). Press both switches then go to the blue key compartment, where a new chamber holding shotgun guys and one to three arch-viles has opened, then collect the megasphere and step inside the skin-box and press all four eye switches inside it.

After pressing the last switch, drop down the hole that opens at the south end of the room to face a large devil-figure. After a few moments, the figure's mouth will open and begin firing mancubus fireballs and revenant missiles at you. Fire rockets into the center of the mouth to hit an unseen Romero's head - once it dies, the level will end.
