A long, dark corridor lined with cages and shackled monsters stretches into darkness. It branches midway: left is a flesh elevator behind a yellow gate, right is a twisting staircase leading to a large cavern where the yellow skull key can be found. Continue straight ahead to find the blue door and a small lounge with several helpful items (and your first encounter with Dr. West himself, who taunts you before teleporting away).
The yellow elevator leads to a hideous fleshy room and an exit switch guarded by an Eating Machine boss.
Boss: Eating Machine
This massive worm takes a lot of punishment, and when it curls up defensively, the thick hide on its back can deflect any attack you throw at it. From afar it spews a stream of acidic bile, but up close it is far worse, tearing through you like a living chainsaw. Occasionally it makes a dash attack to devour you from a distance.
When the Eating Machine is killed, the door to the exit switch opens.
Other points of interest
The lounge bookshelf has a book switch that reveals a small arsenal and a Ghoul/Spectral Prankster ambush. Behind the Spectral Prankster's hiding place is a secret door to the upper levels of the cavern, where a Topheavy Demon defends the blue skull key.
- In the northwest room, head to the bookshelf at the east end and look for a book that is tucked in. Using it reveals a tome and potassium bombs. (sector 71)
- Head to the north alcove in the northwest room and use the wall to get a health bonus and onto a higher ledge that has shotgun shells and a terror mask. (sector 157)
- Open the blue skull door and enter the hole. The secret is just after the door, with a tome. (sector 211)
